1. From the Staff Links page, Login to PAETEP. Your username is your email address, your default password is your 7 digit code.
2. Click on My Observations on the top right section of the screen.
3. Scroll down to the section called Custom Forms toward the bottom of the screen. Look for the form called Evaluation Completion Form. Under Actions click on View Form.
4. Fill out the information. If you have comments regarding your evaulation, type them in that section. Click on the check box for Electronic Signature which means that you have been advised of your performance status and does not necessarily imply that you agree with the evaluation. When you are done, click Complete Form. Clicking Complete Form means you have reviewed this document and discussed the contents with your manager.
5. When you are done, please remember to Logout by clicking the Logout link on the top right-hand corner of the screen.
