Southwood Student Entry

The Focus Parent Portal will be the method utilized to enter new or returning Southwood students.

 

1. Visit the Focus Parent Portal Registration Page at http://uscsd.org/register.

 

2. If you do not have a Parent Portal account, click on I DO NOT have an Account Registered on the Parent Portal.

 

 

3. Enter the required information and click Submit to create your Parent Portal account.

 

 

4.  If you already have a Parent Portal account, click on I have an Account Registered on the Parent Portal but would like to ADD A CHILD.  You will be prompted to log in with your Parent Portal account.  

 

5. Click on I would like to APPLY FOR ENROLLMENT for a new child to start the registration process.

6. Enter the required information regarding the child, select Southwood Registration as the form type, and click Begin Application to access the New Student Registration Form.

 

 

7. All fields within the registration form requiring a response are marked with a red asterisk (*). The Resident School District must be populated and can be found in the drop down menu by typing in the name of the district, the AUN number, or by scrolling through the options.

 

 

8. The student's Ethnicity and Race must be completed.  For example, if the Single Ethnicity selected is White, then the Race option of White must be Yes.

 

 

9. Some districts have a contract for services with Southwood, and this is necessary information for proper reporting and billing. Some students arrive at Southwood due to placement by an agency or a court decision.  The Southwood admittance date is the first date that the student became housed at Southwood, regardless of whether education has begun. 

 

 

 

10. Once these fields have been completed, the next step is to provide information for the parent/guardian's address and contact information.  Complete the Address Information for the first person and click on the green Save Address button.

 

 

11. If you need to correct the saved information, the Saved Addresses can be changed by clicking on the blue Edit button. If everything is correct, the option is available to add new addresses or move on to complete the parent/guardian's contact information by clicking on the green Add New Contact button.

 

 

 

 

12. Contact Detail includes the ability to choose the type of phone number (work, cell, home, etc.).  The phone number must be entered using dashes. When finished, there is the ability to Add More Contact Details, or simply Save Contact.

 

 

 

 

 

 

13. The form allows the user to continue to add as many contacts as needed.  When all data is accuracte and complete, click on Submit and Finish.
